You choose from three builds. The 10.5 by 10 inch bar mitzvah bag is the boy's size. The 14 by 14 large talis bag is the adult standard, and the 16 by 16 extra large is what a heavy wool tallis really wants, particularly if a set of tefillin has to share the space with it. Men who carry a tallis katan or a gartel in the same bag should take the largest.
The design number carries across the Malchut range, so a siddur cover or bencher in the same design can be requested later. That is the practical reason to note #5045 down rather than trusting to memory.
